*A photo of identity of the head of the family.

*A copy of the marriage certificate.

*Copy of birth certificates of the two spouses.

*Copy of birth certificate of the family members.

*Copy of the death certificate of the spouses, when the booklet is delivered to the mother.

*An expedition of the divorce judgment for the mother who has the custody of children.

*Charges on the family record book.

Note: When the demand for a family record book is concomitant to the conclusion of the marriage certificate in front of the civil status officer, the same documents are required for the initial establishment of the family record book (for the first time).
